Evaluating app measuring frailty in patients prior to discharge from hospital

Buckinghamshire Healthcare NHS Trust is evaluating implementation of a free app which generates a frailty score (created by the Acute Frailty Network) as part of its three-year frailty strategy, which will involve a large patient pathway restructure. The app will help clinicians work towards the strategy’s ‘3Rs’ of Frailty: Recognise, Record, React – and provide better patient care through earlier identification and better management of frailty in the community.

Frailty impacts a huge number of patients, and it is hoped that providing them with earlier diagnosis of frailty and an accompanying care plan will lead to fewer presentations at GP surgeries or hospital, better patient outcomes and more efficient use of healthcare resources.

The Oxford AHSN will provide ad hoc project management and health economics support to show the clear system benefits of the free app and pathway redesign.
